What to send Space Maintainers Laboratory?
Please send upper and lower models poured in stone. Also please send a bite record taken with the mandible advanced 60%-70% of the total AP range of motion.
What is the turn around time?
Space Maintainers Laboratory has a standing policy to send your appliance back to you within two weeks. That is two weeks form the date of impression to the date of delivery. Please call if you need a quicker turnaround and we will assist you any way we can.
What will you receive from Space Maintainers Laboratory?
You will receive the upper Lamberg Sleep Well Appliance member, which fits on the palate, along with a lower full arch invisible retainer. The Lamberg Sleep Well Appliance will advance the mandible and the invisible retainer will prevent any flaring of the lower incisors.
How do I send my cases to Space Maintainers Laboratory?
Space Maintainers Laboratory will supply you with boxes, UPS mailing labels, Rx forms, and bubble wrap. Give us a call (1-800-423-3270),or visit our website (
www.appliancetherapy.com) to order these items.
Please carefully wrap each model with a piece of bubble wrap and secure with a rubber band or tape. Make sure the models are identified with the doctor's last name as well as patients first and last name. Apply the label to the outside of the box and it will arrive at our P.O. box within a few days. If you are requesting an expedited service please ship UPS or Fed-X to our street address.
